My 1996 does the same thing, at 3K miles. What I've found most effective is to double stroke the shift lever while holding the clutch in. In other words, instead of trying to go into fifth in one upstroke, gently do two stokes, clutch disengaged. I've had no problem with what you mentioned like I did when I bought it. The only thing with this method is that you can't shift fast from the fourth to fifth, it's kind of a "delayed" shift. Give it a try and see what you find...
